But three qualifiers change what it means in practice.

Modest. GH begins rising as insulin falls, but you’re in the shallow end of the response curve.
Randomized trials of 16:8 time-restricted eating show real body composition benefits — one found a 2.46% reduction in body mass and an 8.65% drop in fat mass with no loss of lean tissue. But those trials generally don’t demonstrate a specific hormonal advantage over standard calorie restriction.
A 2020 randomized trial in JAMA Internal Medicine found time-restricted eating produced changes broadly comparable to conventional dieting. The benefit is real. The mechanism probably isn’t primarily a GH spike.
24 hours
Now it gets substantial. A 24-hour fast produced roughly a five-fold increase in GH secretion in healthy humans.
A 2019 crossover trial found mean GH rising from 0.73 ng/mL to 2.4 ng/mL over the same window — roughly a tripling.
There’s a finding here worth pausing on. Research on water-only fasting found the GH increase happens independently of weight loss, and that people with lower baseline GH saw the largest relative increases.

Less than you’d fear. Fasting research in older adults found roughly a four-fold increase in 24-hour GH production after two days — with GH per pulse doubling.
Absolute levels were lower than in young men. But the fold-increase was comparable, meaning the mechanism still works on an aging axis.
That’s genuinely good news if you’re 50 and wondering whether any of this applies to you.
Why Fasting Raises GH — And the Part Most Articles Get Wrong
The standard explanation you’ll read everywhere goes: fasting lowers insulin and raises ghrelin, and both push GH up.
The first half is solid. The second half is shakier than anyone admits.
Insulin is the main driver
Insulin is growth hormone’s direct antagonist. When insulin is elevated, GH release is suppressed; when insulin falls, that brake comes off.
Fasting removes the brake more completely than anything else you can do without a prescription. That’s why fasting outperforms simple calorie restriction for GH — very low-calorie diets don’t reproduce the same response.
It appears to be the absence of food, not merely the deficit, that triggers it.
The ghrelin story is contested
Ghrelin is a genuine growth hormone secretagogue — it binds the GHSR-1a receptor and stimulates GH release. That’s well established pharmacologically.
But whether it drives the fasting response in humans is another matter.
A 2022 study in the Journal of Clinical Endocrinology & Metabolism looked directly at this. Twenty-four-hour fasting produced substantial GH hypersecretion that was ghrelin-independent.
Total ghrelin didn’t reliably change across the group. Some participants went up, some went down, with large individual variability.

What the surge is actually for
This is the reframe that makes everything else make sense.
The fasting GH surge is not a performance upgrade your body grants you for discipline. It’s a survival adaptation.
Growth hormone during a fast does three jobs: it drives lipolysis so you burn stored fat, it maintains blood glucose, and it spares protein so you don’t cannibalize muscle for fuel.
Your body isn’t rewarding you. It’s managing a famine.
Once you see it that way, the next section stops being surprising.
The IGF-1 Paradox: The Part Nobody Tells You
If you take one thing from this article, take this.
Growth hormone doesn’t do most of its work directly. It signals your liver to produce IGF-1, and IGF-1 is what drives the anabolic effects everyone actually wants.

Why it happens
The mechanism is well-mapped and it comes back to insulin again — but in the opposite direction from what you’d expect.
Insulin controls how many growth hormone receptors your liver expresses. Higher portal insulin makes the liver more GH-sensitive; low portal insulin makes it less sensitive.
So during a fast, insulin drops. Your pituitary shouts louder, and simultaneously your liver turns down its hearing aid.
There’s a second contributor. FGF21, a hormone induced by fasting, directly causes GH resistance by reducing activated STAT5 in the liver — the main relay for GH’s message — which cuts IGF-1 gene expression.
Why your body does this deliberately
This isn’t a design flaw. It’s the point.
IGF-1 lowers blood sugar. During a famine, the last thing you need is a hormone driving glucose down and pushing energy into building new tissue.
So your body raises GH — which mobilizes fat and protects muscle — while suppressing IGF-1, which would otherwise burn scarce fuel on construction projects.
It’s an elegant piece of metabolic triage. It’s just not what the supplement ads are implying when they quote the five-fold figure.
So Does the GH Spike Build Muscle?
Direct answer: no, not by itself.
I know that’s not what you wanted to hear, and I know it contradicts a lot of content. Here’s the reasoning.
Protective, not anabolic
GH’s muscle effect during a fast is protein-sparing. It stops you breaking down lean tissue for fuel.
That’s genuinely valuable — it’s why fasting preserves muscle better than a plain calorie deficit. But preserving is not the same as building.
For actual hypertrophy you need the IGF-1 arm of the axis working, and that’s precisely the arm fasting suppresses.
The two-phase reality

Don’t misread this as dismissal — I use a daily eating window myself and I’d recommend it to most men. It just works through different mechanisms.
- Improves insulin sensitivity, which lowers the chronic insulin that suppresses GH the rest of the time
- Reduces body fat, and visceral fat is a direct GH suppressor
- Simplifies eating, which helps people hold a deficit without tracking
- Produces modest acute GH elevation at the tail end of each fasting window
That’s a solid list. Notice that the biggest items are indirect — they remove suppression rather than driving secretion.
The realistic framing
Think of it this way. Daily time-restricted eating slowly clears the roadblocks to your natural GH output.
Occasional longer fasts produce the dramatic acute spikes.

Which Fasting Protocol Fits Which Goal
There’s no single best protocol. There’s a best protocol for what you’re actually trying to do.
If your goal is sustainable fat loss: 16:8, daily. Highest adherence, real body-composition results, minimal downside.
If your goal is maximum GH response: a 24-hour fast, once or twice a month. Dinner to dinner. This captures most of the acute benefit without the risks of extended fasting.

Who Should Not Fast
This section is short and I’d rather you read it than skip it.
Do not fast without medical supervision if you:
- Take insulin or sulfonylureas — fasting can cause dangerous hypoglycemia
- Have a history of disordered eating, at any point in your life
- Are significantly underweight or already in a heavy deficit
- Are pregnant or breastfeeding
- Have a history of gallbladder problems
- Take medication that requires food, or that affects blood sugar
Stop a fast immediately if you experience dizziness, heart palpitations, or confusion. Those aren’t signs of a good fast. They’re signs to eat.
